QDRO Procedures

The Procedures provide additional information on how QDROs are processed, such as how an account hold is placed, how the Plan interprets ambiguous language, and what types of orders are accepted.

Sample QDRO

You may, but are not required, to use the Plan’s fill-in-the-blank Sample QDRO for your order.  The Sample QDRO uses language that’s pre-approved by the Plan.

Authorization to Review Proposed Order Form

If you’d like the Plan to pre-approve your order before it’s submitted to the court, the Participant must complete and send us the Authorization to Review Draft QDRO form.  The Plan doesn’t require you to have an order pre-approved by the Plan.

Authorization to Review Draft Order Form

If both the Participant and Alternate Payee would like to waive the 30-day objection period after an order is preliminarily approved as a QDRO, they must complete and send us this form.

Important: The Plan will only accept this form if it is completed after the preliminary determination letter is sent.
